    Tata Chemicals fined over £1 million following contractor’s death at UK plant 10 JUNE 2024Tata Chemicals Europe Limited has been fined after a young father died following an incident while erecting a scaffold tower in a chemical plant in Northwich, Cheshire. Michael Densmore died following complications to a wound he sustained after his right foot slipped into a trough containing liquid chemical calcium hydroxide – more commonly known as ‘milk of lime’ – causing chemical and thermal burns.The 37-year-old father-of-four was one of a number of scaffolders employed by Altrad NSG to erect scaffolding at Tata’s Lostock Hall site. On 30 November 2016, during the course of this work, Densmore stepped over a trough which contained milk of lime – which had been heated to approximately 90 degrees centigrade. His right foot slipped on an unfastened lid covering the trough, resulting in him sustaining chemical and thermal burns to his foot and ankle.He was airlifted to Whiston Hospital burns unit, where he received specialist treatment and underwent surgery on 8 December and was discharged just over a week later.However, on 3 January 2017, while at home, he suffered a haemorrhage to his right foot and was taken to hospital following a 999 call, but tragically died.The Health and Safety Executive (HSE) investigation found there was no permit in place for hazardous work in a live chemical plant. Little proper thought had been given to the risks involved by those responsible for ensuring staff safety. As far as the scaffolding team was concerned, there was no clearly understood plan to address these risks.Densmore had only received a brief induction when he started work on the site some months before. Crucially, he had not been warned that there would be chemical product flowing through the plant and that the lids to the trough had not been properly sealed.Tata employees had been seen working on or near the troughs and there were no visible warning signs in place.It also found that there had been previous prosecutions of Tata Chemicals Europe relating to health & safety failures at Lostock Hall and nearby Winnington Lane.Tata Chemicals Europe Limited pleaded guilty to breaching Section 3(1) of the Health and Safety at work etc. Act 1974. The company was fined £1.125 million and ordered to pay £60,603.54 in costs at Chester Crown Court on 5 June 2024.“Michael should not have been put in this unsafe working situation and should have been warned about the dangers of stepping over the troughs and that they were still in operation.“Companies should learn the lessons from this incident if they have staff or contractors working in a similar environment and be aware that HSE will not hesitate to take appropriate enforcement action against those that fall below the required standards.”#riskassessments #engineeringcontrols #HSE #sonsucontrolsMore information...https://lnkd.in/dNbpX4g2

    NTSB: Rule Violation Contributed To Fatal Midair Collision At EAA AirVenturehttps://lnkd.in/gXyVHpN9The National Transportation Safety Board (NTSB) released its final report on last year’s fatal midair collision at EAA AirVenture, concluding that the crash occurred when a gyroplane violated event rules by making a prohibited 360-degree turn and collided with a helicopter.According to the NTSB report, both pilots attended a rotorcraft briefing where event coordinators told gyroplane pilots to stop performing 360-degree turns and spirals while in the traffic pattern. Flight track data, witness testimonies, videos and the damage to the aircraft indicated the gyroplane collided with the left side of the helicopter while performing a 360-degree turn while on the base leg of the visual approach. At the time of the accident, the helicopter was positioned behind the gyroplane in the traffic pattern and was also on a base leg from the south.The helicopter impacted the ground and came to rest upside down, igniting a post-accident fire, while the gyroplane collided with a parked, unoccupied airplane. The pilot and passenger of the helicopter both died while the gyroplane pilot and passenger sustained serious injuries.     Conveyor belt at one of Brazil’s busiest ports closed due to explosion risk. Apr 24The Port of Paranagua, one of the busiest sugar and grain export terminals in Brazil, has had one of its conveyor belts shut down after authorities said there was a risk of explosion. Although the conveyor was closed on 1 April, exports at the port have continued with five more conveyors continuing to operate.Port of Paranagua- Image: ShutterstockWhen operational, the conveyor belt is used to export around 20 million tons of grain a year via the Port of Paranagua. Authorities from Brazil’s Ministry of Labor found serious maintenance issues and excessive dust accumulation which could lead to an explosion.While the closure of the conveyor has not prevented the export of sugar and grain, there have been slight delays to ship loading at the port. The order to close the conveyor belt took effect at the beginning of April, however authorities have not said when it can become operational again.Located in the city of Paranagua in the Brazilian state of Paraná, the port is the second largest in the country in tonnage and the third in container shipping. The two employees who were injured were transported to hospital for treatment but are expected to make a full recovery.Phillips 66 said an investigation into the incident would begin immediately to determine the circ*mstances around the incident.The Borger Refinery is located in Borger, Texas, in the Texas Panhandle, approximately 50 miles north of Amarillo, Texas. Refinery facilities include crude distilling, naphtha reforming, fluid catalytic cracking, alkylation, hydrodesulfurization, and delayed co*king units. The refinery produces a high percentage of transportation fuels, as well as fuel-grade petroleum co*ke, NGL and solvents. Refined petroleum products are distributed to customers in West Texas, New Mexico, Colorado and the Midcontinent region by company-owned and common carrier pipelines.

    US Department of Labor alleges pesticide manufacturer illegally terminated employee who filed safety complaint26 MARCH 2024The US Department of Labor has filed a federal court complaint against a pesticide and agricultural chemical manufacturer in Georgia after the company allegedly terminated a worker's employment in retaliation for filing a safety and health complaint with the department.Filed in the US District Court for the Northern District of Georgia, the complaint proposes Avenger Products LLC and its parent company Kittrich Corp. pay the former customer service manager back pay, compensatory and punitive damages to address lost wages, other financial losses and emotional distress related to their termination and forbids the company from future retaliation permanently."Employers are required to comply with all applicable anti-retaliation laws, including those meant to ensure worker protection when raising health and safety concerns in the workplace under the OSH Act," said Regional Solicitor of Labor Tremelle Howard in Atlanta. "When employers retaliate against employees, the Department of Labor will not hesitate to use all enforcement tools available to stop this conduct."OSHA alleges that Avenger Products violated the Occupational Safety and Health Act by ending the worker's employment because they exercised their rights protected under the act. A whistleblower investigation by the department's Occupational Safety and Health Administration found that the employer terminated the employee after they made complaints about safety hazards associated with chemical exposures in the workplace."The employer's actions toward this worker and its response to concerns about the potential chemical exposure hazards to their workers are alarming," said OSHA Assistant Regional Administrator Lily Colón in Atlanta. "Retaliation by employers like Avenger Products against employees who exercise their protected rights under the law create in a chilling effect on the willingness of other employees to cooperate with federal safety investigations. This creates a culture of fear and a workplace rife with potential dangers. This year has been terrible. We have been let down by our non-profit TEL international, by the Ghana government and also by some very close folk we thought were our friends and brothers. I am sorry everyone, I let you down. All the money we raised for Buduburam was a short-lived respite. Today their homes, the local market, ability to get jobs by hopping on a bus from the main highway, all is lost. These refugees have been asked to return to their own countries but were not given any transitionary assistance.My own personal donation to Techiman Action School in memory of my child has been tainted with discord with TEL and their refusal to support Buduburam. Their focus is religion first. I am walking away from TEL and Ghana forever. My efforts only brought disillusionment. I feel poorly about not being able to amplify your contributions into a better future. This world cares about religion, politics and border sovereignty, people come a distant fourth of fifth.Emmanuel Jallah, my boy - I shall try to get you to Canada. One man has limited resources and can only do so much.

    Hydrogen safety in oil refineries and beyond - Andrzej Janowski MSA23 FEBRUARY 2024#H2 #classifiedareas #hazardousareas #intrinsicsafety #flameproof #notsoeasy Hydrogen, however, has been used safely for decades in oil/gas refineries in critical process applications where its hazards are well known, it is safely monitored, and there are lessons to be learned.Hydrogen gas characteristicsFire, explosion, and asphyxiation are the primary safety considerations associated with handling hydrogen gas, especially considering its wide flammability range of 4-77% of volume in air. The main areas of risk can be categorised as shown in Figure 1. In oil/gas refineries, with their complex mazes of tanks, pumps, and piping, H2gas leaks are a serious hazard across the facility.Hydrogen gas is invisible to the eye and has no odour, making it undetectable by human senses. H2is lighter than air and can drift outside. It is commonly understood that in confined inside areas it will rise upwards to ceiling level displacing oxygen. However, pressurised outdoor hydrogen gas leaks can be hard to detect as the gas jet direction can be unpredictable, making it difficult to detect in refinery spaces where accumulations cannot occur.Typical refinery applicationsOil refineries are large producers and consumers of H2gas. Hydrogen plays a pivotal role in many refining operations, from hydrocracking – reduction of heavy gas and gas oils to lower molecular weight components – to treatment of gas streams, and catalytic reforming.In catalytic reforming processes, H2is also used to prevent carbon from reacting with the catalyst to maintain production of lighter hydrocarbons and to extend catalyst life. Not surprisingly, refineries use large volumes of H2that are sometimes produced on site or purchased from H2production facilities.In oil/gas refineries, the primary hazards associated with H2gas include respiratory ailments (oxygen deficiency), component failure, ignition, and burning. Although a combination of hazards occurs in most leak incidents, hydrogen’s primary hazard is production of a flammable mixture that can lead to fire or explosion. As H2’s minimum ignition energy in air at atmospheric pressure is approximately 0.02 mJ, H2is easily ignited.Layered gas and flame safetyIn order to address hazards posed by H2, fire and gas detection system manufacturers work within the construct of layers of protection to reduce hazard propagation. Using such a model, each layer acts as a safeguard, preventing the hazard from becoming more severe.Key challenges for oil/gas refineries handling or storing hydrogen, with supporting hydrogen production plants, include detecting leaks outside, where the gas can’t accumulate, and installing detectors appropriately within different risk zones. A robust and layered strategy for fire and gas detection is needed
